Community Access Supports

When it comes to Community Access Supports, our main goal is to ensure that individuals can maintain their independence and stay in their own homes while avoiding social isolation.

At Reachability Australia, we understand that this can be a challenging task, but we are always up for the challenge! Our team works hard to deliver these supports one-on-one with each of our clients in a creative and innovative manner.

We understand that everyone's interests and needs are different, so we cater our services to each individual. Our dedicated, highly-skilled staff is committed to delivering excellent care to ensure that our clients can live independently and happily within their community.

We believe in the importance of integrating with the community and providing access to recreational activities, social opportunities, and developing skills to increase independence.

Community Access Programs

Community Access Supports are designed to help people with various activities outside of their home. Our aim is to provide our clients with the necessary support to enable them to run errands such as grocery shopping or attend medical appointments.

Our services also include helping individuals navigate to a friend's or relative’s house or participating in recreational activities of their choice.

We provide access to concerts, cultural group meetups, social events, and any other preferred locations that our clients wish to go to. Our supports are goal-based, and our team of support workers are always happy to assist our clients to achieve the goals they have set for themselves.

On the other hand, Day Programs are specially designed to help our clients build independent living skills and to encourage their participation in community-oriented activities. Our clients attend the Day Programs about three to five days a week, and we offer activities that allow them to improve their skills. These life skill development activities include cooking, shopping, and communication, among others.

The Day Programs are group-based, which means that several clients attend at any given time. We offer a set program schedule, and community-based activities are generally delivered at the same location. Though our clients have the option to choose from 1-3 activities on any given day, they're located in different rooms, and some participants may feel limited by the set program and desire a more personalised approach to support.

Funding for Community Access and Day Programs are both funded from ‘Core – Assistance with Social, Economic and Community Participation’ on your NDIS Plan. The difference is generally the prices charged for each service which can be found in the NDIS Price Guide.

Reachability Australia offers Community Access with our friendly team of support coordinators and support workers dedicated to working with you to identify and implement personalised community access supports to suit your needs and interests.

Yes, absolutely! A Companion Card is available for people living with a significant and permanent disability that allows your Support Worker/Carer/Companion to obtain a second ticket, free of charge, at selected venues and facilities around Queensland. If you think you may be eligible, have a chat with the team at Reachability Australia who can help you to apply.

We sure do. At Reachability Australia, we support you to access the community on your terms. For some of our clients, this includes the want to be supported by an ‘unbranded’ support worker who looks more like a companion or friend to fellow community members.

It’s your choice whether you’d like your support worker to wear a uniform or drive a company (branded) car for example. Whatever your preference is, have a chat with our team about how we can make your experience more comfortably and uniquely your own.

If you meet the eligibility criteria, there are two ways you can apply for NDIS funding. You can either call NDIA on 1800 800 110 to make a Verbal Access Request (VAR) or fill out a paper Access Request Form (ARF).

  • What documentation is required?
    Completed Access Request Form (ARF) – ask your Local Area Coordinator, Early Childhood Early Intervention partner (if your child is under 7), your local NDIA office or call NDIA 1800 800 110 to receive a form.
  • Age and residency evidence – Tip: If you’re registered for Centrelink, you can give the NDIA consent to use your Centrelink record.
  • Disability evidence and how it impacts your everyday life – Tip: Ask your healthcare professional to complete Part F of ARF or an NDIA Supporting Evidence form.
  • Any existing reports or assessments.

A thing to remember…

When NDIS participants turn 65, they have the option to stay in the scheme or apply to receive support through aged care services.
